A resident may be visited by no more than two general visitors at a time.
All visitors seeking entry onto Lakewood Center’s campus must be able to pass a COVID-19 screening questionnaire, including a temperature check. Please review below for exclusionary criteria for general visitation:
  1. Any person infected with COVID-19 who does not meet the most recent criteria from the CDC to end quarantine.
  2. Any person showing, presenting signs or symptoms of, or disclosing the presence of a cough, fever, shortness of breath, sore throat, respiratory infection, loss of taste or smell, muscle pain or chills, fatigue, malaise, diarrhea, nausea, vomiting, headache, red eyes or any other COVID-19 symptoms identified by the CDC.
  3. Any person who has been in contact with any person(s) known to be infected with COVID-19, who does not meet the most recent criteria from the CDC to end isolation.
All visitors must immediately inform the facility if they develop a fever or symptoms consistent with COVID-19, or test positive for COVID-19 within fourteen (14) days of a visit to the facility.
Each visitor MUST:
  1. Check-in at the entrance to the Health Center for screening prior to entering any other area of the Lakewood Center campus;
  2. Wear a face mask and any required PPE at all times and perform proper hand hygiene. If Lakewood is experiencing a PPE shortage, visitors may be required to bring their new PPE for each visit to wear while visiting Lakewood Center.
  3. Complete required training to include infection-prevention and control education of visitors on use of PPE, use of masks, hand sanitation, and social distancing, as well as Lakewood Center’s visitation policy.
  4. Visits will take place in a designated area. Designated areas will meet the following criteria:
    1. Outdoor visitation spaces that are protected from weather elements, such as porches, courtyards, patios, or other covered areas that are protected from heat and sun, with cooling devices if needed.
    2. Indoor visitation spaces for residents in a room that is not accessible by other residents.
    3. Visitors will not be permitted to visit with residents in their rooms/apartments.
  5. Maintain social distance of at least six feet with staff and residents for the duration of the visit.
