Application Process

What Happens When You Apply?

BEHIND THE SCENES OF THE APPLICATION PROCESS

Applying to Lakewood involves submitting a general inquiry that includes your (or your loved one’s) full name, contact information, and diagnosis. Our admissions team will review your inquiry and reach out to continue your application, including assessing the medical history and background of the prospective resident. Applicants undergo an evaluation process to determine if Lakewood Center's services align with their treatment goals and diagnoses. On average, our admissions process can take approximately 1-3 weeks depending on each individual’s clinical and medical history.

Potential residents will be asked to provide:
Clinical history & application for admission
In person or virtual interview
Clinical records from previous placements
Psychiatric provider records
History and physical evaluation (or complete within 30 days of admission)

Please contact us if you have any questions.
